ENROLLMENT PROCESS

For both returning and new to the district students

Parent/guardian enrolling student must present the following information upon completion of registration:

Required for enrollment:

• Parent/Guardian ID (Any official photo ID)
• Proof of Guardianship/Custody (if applicable) if a child is not living with his/her parents or if a child lives with only one parent, proof of legal guardianship or custody is required.
• Child’s original Birth Certificate
• Child’s Report Card, withdrawal form from previous school district and/or high school transcript.
   

The following can be obtained within 30 days of enrollment:

• Proof of Residence (MUST BE PRESENTED ANNUALLY)
  • Current Utility Bill (water, light, or gas bill with parents name and address)
  • Lease agreement/Renter’s contract
  • Notarized letter of residence (if applicable) if parents/students are living with a friend or relative who is the homeowner of a residence of the District.
  • District affidavit of residence.
• Child’s Immunization Record (Must be up to date)
• Child’s Social Security Card (optional)

PROCESO DE INSCRIPCIÓN

Para estudiantes que regresan y nuevos en el distrito.

El padre/tutor que inscribe al estudiante debe presentar la siguiente información al completar el registro:

Requisitos para la inscripción:

• Identificación del padre/tutor (cualquier identificación oficial con foto).
• Prueba de Tutela/Custodia (si aplica) si el niño no vive con sus padres o si el niño vive solo con uno de los padres, se requiere prueba de tutela o custodia legal.
• Acta de nacimiento original del niño.
• Boleta de calificaciones del niño, formulario de retiro del distrito escolar anterior y/o expediente académico de preparatoria.

Los siguientes documentos pueden obtenerse dentro de los 30 días de la inscripción:

• Comprobante de residencia (DEBE PRESENTARSE ANUALMENTE).
  • Recibo de servicio actual (recibo de agua, luz o gas a nombre del padre y con la dirección).
  • Contrato de arrendamiento/acuerdo de renta.
  • Carta de residencia notariada (si aplica) si los padres/estudiantes viven con un amigo o pariente que es el propietario de una residencia dentro del distrito.
  • Declaración jurada de residencia del distrito.
• Registro de vacunación del niño (debe estar al día).
• Tarjeta de Seguro Social del niño (opcional).

Yes, PSJA ISD is an OPEN ENROLLMENT school district. You may attend our schools, PK-3 to 12^th grade, regardless of place of residence. You may apply online at www.psjaisd.us/register and select your preferred school. If you need assistance selecting a school, you may contact our Community Engagement Department.

The parent or guardian of the student must provide one of the following items listed below to the school during registration annually:

1.  Most recent utility bill such as water, light, or gas with the parent's/guardian's name and address.

2.  Mortgage statement or deed with parent/guardian name.

3.  Lease agreement with parent/guardian name.

4. Rental agreement with parent/guardian name.

5.  District Notarized Affidavit of Residency if the parent/guardian live with another person.
